Allied Health Professional: Extended Scope of Practice
Permanent – Part time/Full time, Up to 40 hours per week

Colac Area Health is seeking applications for a passionate Allied Health Professional to join our dynamic team; extending scope of practice in a transdisciplinary model of care. Selection Criteria |
Essential: Registered clinician with Australian Health Practitioner Regulation Agency (AHPRA) or eligible for AASW/DA/SPA membership. Essential: Demonstrated experience or interest in working as a transdisciplinary Allied Health Professional. Essential: Demonstrated clinical expertise to assess functional, cognitive and psychosocial needs of person and family and determine appropriate intervention, care coordination or case management in aged, hospital or community services. Essential: Advanced interpersonal, written and verbal communication skills, including ability to build strong professional partnerships. Essential: Sound organisational ability in time, caseload management and the ability to work autonomously. Essential: Excellent IT, data analysis and report writing skills. Essential: Applicants must be able to work flexibly across the week according to the needs of the service. Essential: Ability to initiate, influence and drive a coordinated team approach to patient care, particularly for those with complex health needs . Essential: Excellent knowledge of/or the ability to develop an excellent knowledge and working relationship with extensive community resources within the region. Essential: Demonstrated commitment to ongoing professional development and education. Desirable: Current Class C Driver’s Licence. |
