Allied Health Advanced Practitioner GEM@Home - Grade 2 
8 Hours per week until 30/06/2024

 

Colac Area Health is a values based organisation providing a unique integrated health service comprising of over 500 employees, providing Acute Care, Aged Care, Community and Allied Health Services.

 

The GEM at Home Allied Health Advanced Practitioner (AHAP) role is held by an experienced Allied Health Therapist, operating at an advanced scope of practice to perform additional tasks identified as being transferable between professionals to deliver high quality care to Geriatric Evaluation and Management (GEM) at Home patients.

 

What’s required? 

  • Registered Clinician with Australian Health Practitioner Regulation Agency (AHPRA) or eligible for AASW/DA/SPA/ESSA membership.
  • Demonstrated clinical expertise in assessment, care coordination or case management in aged, hospital or community services.
  • Demonstrated clinical expertise or willing to complete competency training to deliver transferable care across nursing, occupational and physiotherapy scope of practice.
  • Ability to communicate effectively with a wide range of people.
  • Skill in the evaluation and implementation of evidence-based practice and modification of service delivery.
  • Experience delivering care in the home environment.
  • Experience providing patient centred care and working collaboratively with clients
  • Ability to proactively contribute to and adapt to internal and external change.
  • Ability to differentiate, accept and exercise appropriate mechanisms of internal and external communication and accountability.
  • Ability to think conceptually and a commitment to innovation.
  • Current Victorian Driver’s Licence and Police Check
